Warfarin Case 5: Beth Novate

This question requires exam stationery. You can use your own university stationery or find ours here.

It is 28/03/2016. You are an FY1 on call and have been asked to prescribe warfarin for a patient you have not yet met. The patient is Beth Novate, (hospital number: MS544908, date of birth: 4/9/65) who was admitted due to worsening Angina. Since admission it has been noted that she is in Atrial Fibrillation. She had a TIA last year and has been worried about having a stroke ever since. Her current medications include GTN Spray, Amlodipine and Simvastatin.

She was not anti-coagulated after the TIA, but after collaborative decision making, it is now decided that she is to begin warfarin treatment to reduce her risk of stroke.

Her INR has been measured since this decision has been made:

24/03/2016 INR – not measured

25/03/2016 INR – not measured

26/03/2016 INR – 1.6

27/03/2016 INR – 1.6

28/03/2016 INR – 1.9

Using these readings and the Warfarin algorithm, please fill in the Warfarin chart below and prescribe her next dose accordingly.
