Warfarin Case 2: Perrie Tonism

It is 28/03/2016. You are an FY1 on call and have been asked to prescribe warfarin for Perrie Tonism, a lady on ward 11. Her hospital number is MS338451, and her date of birth is 25/4/41. Perry had been admitted to hospital following an infective exacerbation of COPD. Since admission it has been noted that she is in Atrial Fibrillation. She is known to have Hypertension and COPD. Her current medications include: Metformin, Gliclazide, Symbicort inhaler, Salbutamol inhaler, Amoxicillin and Prednisolone. After careful deliberation between Perrie and the medical team, with support from the ward pharmacist, all are in agreement that it would be beneficial for Perrie to begin warfarin treatment to reduce her stroke risk.
