End of Life Documentation 1: Chester Drain

You are the SHO on the respiratory ward at Medisense General Hospital, where Mr Drain (DOB 21/10/1941, NHS# B729341, home address 14 Moore Close, Strawbridge) has been admitted for his 4th Infective Exacerbation of COPD in 3 months. You see from the notes that his GP (Dr Park, Strawbridge Medical Practice) has noted his significant decline in his clinical condition and performance status over the past 6 months. He lives alone and has no surviving family. He has been prescribed antibiotics and steroids for the infection, but at the MDT, a decision is made that he would not be appropriate for resuscitation. One of the nurses adds that Chester himself says he has “had enough of hospitals”, which you discuss with him and he confirms he would not want to be resuscitated if he stopped breathing. There are no current plans for discharge or transfer, and Chester has capacity. Complete the appropriate documentation for this decision.
