Discharge Prescribing 5: Ray Naud

You are the F1 on the cardiology ward. Ray Naud was admitted with progressive SOB and peripheral oedema. He was diagnosed with congestive cardiac failure, and has been steadily improving. He is now well enough to go home. Having been previously fit and well, other than poorly managed hypertension he has a lot of new medication to be discharged home with and you have been asked to prescribe them. His details are as follows:
